Rivertown Transitional Care LLC (RTC) is a Florida Agency for Persons with Disabilities (APD)-licensed group home located in Blountstown, Florida. We provide Standard Residential Habilitation and person-centered residential supports for adults with intellectual and developmental disabilities.

At RTC, our mission is to provide a safe, supportive, structured, and home-like environment where individuals are treated with dignity and respect while being encouraged to develop greater independence and participate meaningfully in their community.

Our dedicated team provides individualized assistance with daily living skills, personal care, medication support, community participation, and other residential needs based on each individual's unique goals and support plan.
